Our webinar is about individuals grappling with chronic conditions, mental health issues, and social determinants of health needs in our communities. These challenging circumstances and the stories of resilience, compassion, and dedication do not make it to patient charts or care notes. Yet, they are worth hearing, sharing, and celebrating!
